All Medical Records here at Advanced Naturopathic Wellness are online.

• You will receive an email from our office welcoming you to your patient portal. Simply create your own login and password to fill out all of your questionnaires online.
• Please complete these at least 24 hours before your appointment, so that your doctor has time to review them before you arrive.
• If you are unable to login, please call us at (530) 537-2337 and we will be happy to assist you
• Please take a look at our fragrance free policy

If you would like to fill out the forms in the office, please arrive 30 minutes prior your appointment.

New Patient Appointments

Please bring the following to your appointment:

• All of the supplements and medications that you are taking in their original bottles
• Copies of any lab work or imaging reports you have from the past 12 months
• Any other related information

Free 15-Minute Coordination Consultation:

This is an opportunity to meet the doctor to see if she is a fit for you, and if you are comfortable with her approach to helping you. Bring a few specific questions that you would like to have answered. Due to the time frame, the doctor will not be able to go over your entire history, but knowing a little about you will be helpful in your discussion.
